You're technically supposed to have one if the word would be capitalised normally.

Well, I think on this forum, we do have better grammar than the average person. Given there are some people in top set English (which is set partly on a SPaG test) who can't spell and have terrible grammar, but although there are some typos, we do, in general, have good grammar.

For a proper noun, yes. Simply for the beginning of a sentence, I don't believe it's necessary.
